- Get a paper copy of this notice any time.
- Choose a representative to act for you.
- File a complaint with us or with HHS if you believe your privacy rights were violated. (We will not retaliate.)
Our Uses and Disclosures
We typically use or disclose your PHI to:
- Treat you (coordination with other providers).
- Bill for services (claims, billing, eligibility).
- Run our practice (quality improvement, audits, training).
We may also share PHI for public health and safety, as required by law, with oversight agencies, for workers’ compensation, and for other purposes permitted by HIPAA. Full details are in 45 CFR §164.520.
Our Responsibilities
- We are required by law to maintain the privacy and security of your PHI, provide this Notice, and follow it.
- We will inform you if a breach occurs that may have compromised your PHI.
- We will not use or share your information other than as described here unless you give written authorization. You can revoke an authorization at any time.
