Privacy Policy

Who we are

AXIOS Sports Medicine (“we,” “our,” “us”) operates axiossm.com.

What this policy covers

This policy explains how we handle personal information collected on this website and through our business phone/SMS for administrative purposes (e.g., scheduling). It does not replace our HIPAA Notice of Privacy Practices for medical information. See our NPP for how we handle Protected Health Information (PHI). Legal Information Institute

Information we collect on this site

  • Information you submit in forms (name, email, phone, message).
  • Technical data (IP address, device/browser info, pages viewed, cookies/analytics).
  • Basic call/SMS metadata (time, number, message status) when you contact our business line for scheduling.

How we use information

  • To reply to inquiries, schedule/confirm appointments, and provide customer support.
  • To operate, secure, and improve this website (analytics, performance, debugging).
  • To comply with law and protect our rights.

How we share information

  • Service providers that help us run our clinic and website (e.g., web hosting/analytics; our EHR and scheduling system, JaneApp; our business phone provider). Jane hosts PHI securely; we keep PHI within Jane, not on this website or in carrier SMS. Jane - Clinic Management Software
  • We do not sell or “share” personal information for cross-context behavioral advertising.

Cookies & analytics

We use cookies and similar technologies for site functionality and analytics. You can adjust your browser settings to limit cookies.

Texting (SMS) and PHI

SMS is used only for logistics (e.g., scheduling, reminders). Do not send PHI by text. For clinical matters, use phone or Jane’s secure portal. (See SMS/Texting Terms for details.)

Call recording

  • If OFF (default): We do not record telephone calls. Voicemail messages you leave are stored securely.
  • If ON (future): Some calls may be recorded for quality/training—you will be notified and consent obtained at the start of the call (California requires all-party consent). Privacy Rights Clearinghouse
